Financial & Scheduling Policy

  • Deposits: Certain services require a deposit at booking; applied to service if completed, forfeited if late-cancel/no-show (see below).

  • Cancellations/Rescheduling: <48 hours’ notice may incur a fee; repeated late changes may require prepayment for future bookings.

  • No-Show: Fee up to a defined portion of the booked service or loss of deposit.

  • Packages & Credits: Nonrefundable; transferable within household at practice discretion; may be applied to a different service or product upon request; expire 24 months (2 years) from purchase unless stated otherwise.

  • Gift Cards/Retail: Nonrefundable; redeemable for services or products; not for cash.

  • Payment Timing: Payment is due prior to treatment on the day of service.

  • Tips: Never expected for medical services; accepted for spa/esthetic services per practice policy.

Constraints & Clarifications

Nonrefundable Purchases (Refund Policy)

  • All purchases are nonrefundable. At the patient’s request, unused amounts may be applied as credit toward a different service or retail product at Examined Life. Credits are not redeemable for cash.

No Outcome Guarantees

  • Medicine and aesthetics carry variability. No outcome is guaranteed. Estimates of duration or response are educated projections, not promises.

Undesired Outcome – Resolution Pathway

  • If an undesired outcome is suspected, both parties will review standardized photographs (pre/post) and clinical findings.

  • If both parties agree an undesired outcome has occurred, the practice and patient may enter a good-faith corrective plan that represents the best attempt to correct the issue per aesthetic medicine industry standards. This may include additional treatments, product reversal (e.g., hyaluronidase for HA fillers), or modification of the plan.

  • Corrective care focuses on remediation, not refunds. Any costs and timing will be discussed and documented in an addendum to the chart.

  • Corrective neuromodulator or filler treatments must be performed at the scheduled follow-up visit (2–4 weeks). After that timeframe, additional treatments are considered new sessions and may incur cost.

Patient Responsibilities

  • Adhere to pre/post instructions; disclose full medical history and recent procedures; promptly report concerns; attend follow-ups; avoid contraindicated activities/products.

Promotions & Memberships

  • Promotional offers cannot be combined with membership discounts or other special pricing. Patients must choose one type of discount per service.

Privacy Safeguards

  • Verbal consent will be obtained from the patient before any photographs of procedure or outcomes are shared on social media or website.

Minors & Chaperones

For minors, follow state law for consent and scope. For intimate-area procedures, offer a chaperone.
